My index card/other signed during the opening of the Ronald Reagan library and museum on November 4th 1991. Obtained in person by a friend who was there during the ceremony. In attendance also were six former first ladies including Lady bird Johnson. 

Signed by former presidents Richard Nixon, Jimmy Carter and George H Bush. Along with a separate card signed by Gerald Ford.

Unfortunately I did not get the Ronald Reagan signature.
