I not an expert on WH autographs, so expert opinions would be great. Did she always sign with a smiley face?

No, she often signed with just her name and no smiley face, i think she added it later. The size and much of your example is right but I've not seen many slanting to the right when she signed, most I've seen slant to the left. Wait to see what others say.
