I'd recommend shipito, they're reliable, though expensive. I'd say use them only for items that are really worth it to you.

I have a few other items to consolidate into one shipment (which reduces the costs per single item). Hence I wouldn't be able to tell you the additional costs that arise forwarding only one book. My estimation would be USD 30-70 for shipito + import turnover taxes (Einfuhrumsatzsteuer) + carrier fees (Auslagepauschale) after import, adding roughly another EUR 20-30 for such a book.
