Is this the original or one of the carbon copies of this form?

Looks like a copy of the original to me. You would need to see it in person to determine whether it’s live ink or not.

Apparently its the undercopy but an original document not a photocopy the hospital kept the top copy this is the one given to George and is from the estate of Georges PA,i wondered if anyone has ever seen the top copy or if its ever sold at auction

I guess the key word here is “copy”.  I’ve never seen the original top copy for sale but that doesn’t mean it hasn’t been at one time or another
