I've seen these cards signed "To Kayo" over the years.  History For Sale has a bunch still in their inventory.  Most have a photo clipping on the left side and the inscription on the right.

A collector friend suggested that "Kayo" may have prepared these cards with a clipping photo attached and sent them in to the various artists through the mail for signing.  Not sure.  The ink here on Farley Granger looks like the same ink used on the Billie.
