There has been a few times I have used PSA/DNA quick opinion services. I only use it when I am fairly confident that the signature is good and just want an unbiased opinion. The price is reasonable and it keeps the item I'm looking at private. Has anyone run into a situation where PSA/DNA's renders a "likely genuine" quick opinion then submits it to them for a full LOA and it has failed? Or vice versa?

Yes.. Happens all the time, quick opinion is not a reliable indication as to whether or not you will get a full LOA. Especially for higher end items.
As I've noted before, PSA QO has returned "likely not genuine" twice for me on two items that were absolutely without doubt authentic. Now, i think it's likely that had the items been sent in for a full LOA, they probably would have passed them.

The thing is...the person who may have judged for the QO may not be the person that views the item when it is sent in. Perhaps an "intern" views the QO, but hopefully a "pro" views the in-person item.
...Or it could be that if the groundhog sees his shadow it is real!:) Just kidding, but seriously at least with Roger Epperson you know his credentials. At PSA who knows who they even are, unless you can get Steve Grad or another expert it could be some kid out of college. I do assume they have access at least to the same exampler files, but who knows for sure.
What about john reznikoff he used to be a member here who authenticated for psa dna and jsa. Does he still authenticate for them?

I believe that it has happened the other way around as well. That is, the QO result was correct and the in person result was wrong.

You would think they would be just as conservative on a quick opinion as a full blown in-person. I've received several quick opinions from Phil Sears and he says, "an autograph can look good online and not pass in-person, but a bad autograph online should never pass an in-person review". I see his point.

Pay $10 for someone to guess, no thanks. Could be a squirrel working the "Quick Opinion" desk that day, you would never know.

Tony sent me a common Mantle forgery that passed QO the other day. Waste of $ IMHO, much better off asking members here, or find a collector group of your particular interest. I have found that is much more meaningful to me, and it is enjoyable discussing/learning with fellow collectors.
