I'm not positive about this one since I see this was first posted pretty close to 3 years ago but it is in the signed books section under signed fiction, and as you see in the pic says it comes with a signed bookplate. might want to check with the store just in case though since it doesn't say signed in the cart.

I can't remember where I got mine. It was a card signed by Jim and the other author, so it was loose. Can't say if this is the same. The book is a wild trip though and I highly recommend if you are into crazy.
