I used my ShipByMail address when I checked out for this.  Seemed to go through (and Amex accepted the purchase).  The site only offered a billing address option.  No option to add a second address

I couldn't find any 'event' from QBD, only the pre-order purchase of unsigned book.

The ticket confirmation will tell you if you have a signed book, as per the T&C.

I see now why they scuttled the original listing for the book when it was live on Indigo.

I followed KellyEff and setup an account with ShipByMail  which is based in Canada, so shipping (i hope) to US will be not so hyper inflated.
